June 14 & 15 2010 FAYFL Football Clinic

The AYF Medical Clearance form is in addition to the Sports Physical form and must be signed by the licensed physician providing the physical. All physicals must be performed and required forms dated within the playing calendar year (i.e. after Jan 1st of the playing year) .

We are now accepting applications for the 2010 Football Season. Please download this form and turn into your head coach or FAYFL adminstrator. Filling out this registration form does not secure your placement on a team. Every player must be cleared by a given team's head coach and the FAYFL. If you have questions regarding this policy please feel free to contact a FAYFL Executive Officer.

Each player is required to show proof of age through a Birth Certificate. If you don't have a copy of your certificate, a waiver may be granted up until you can get a copy from the state of birth.

This document must be signed by all coaches and turned in to the FAYFL Secretary

This document is must be filled out by all FAYFL coaches or prospective coaches. The Coaches Application is a private document and will only be viewed by the FAYFL Secretary for purposes of background check and coach selections.

Each child is required to maintain a 70% GPA in school to be eligible to play FAYFL football. Copies of your report card will be placed on file.

Every player’s parent must sign and turn in this document as verification of acceptance/responsibility for FAYFL Football Equipment. Please return this signed document to your head coach when equipment is issued.

All particpants are required to obtain a Department of Motor Vehicles (DMV) Child ID (Walker ID), Military ID, or Passport prior to playing in any game. You can locate information on obtaining a DMV ID at www.dmv.state.va.us/webdoc/citizen/id/child_id.asp
